CDC has new PDF forms for reporting shipboard death, illness

The US Centers for Disease Control and Prevention changed the method for reporting death and non-gastrointestinal illness aboard ships destined for a US port of entry.

The Division of Global Migration and Quarantine will no longer accept death and illness reports via the Maritime Illness and Death Reporting System website or MIDRS e-mail after June 10.

Instead, operators are to download and submit fillable PDF forms.

The new reporting method became effective March 10.
