CDC has new PDF forms for reporting shipboard death, illness

The Division of Global Migration and Quarantine will no longer accept death and illness reports via the Maritime Illness and Death Reporting System website or MIDRS e-mail after June 10.

Instead, operators are to download and submit fillable PDF forms.

The new reporting method became effective March 10.
